synthetic weed ( spice in this case? ) will fuck you up hard like stated before, he probably had an allergic reaction to one of the chemicals / the host plant matter as he opened it.
There were no traces of any of the stuff in his system, and an allergic reaction would have showed its own signs on the body.

Turns out it's full of hallucinogenic gas, and his belief of the legends that opening the box kills people caused him to think he was dying, and fall into a psychosomatic near death state.

He had a sudden bout of mortality. It had been an underlying medical condition since the day he was conceived. It just so happened that when he opened the box, his symptoms acted up, causing a premature fatality.
