For example, there may be an intelligence report that says there is an extremist group that the US has been monitoring in Iran, and they have information that they want to send a Iranian man with explosives in his carry on or clothes to blow up the plane in the skies of America. I don't know crap about what a report would actually look like, but even with those simple details, they can narrow it down to a man flying out of Iran or an Iranian flying out of some other country that is around the age of the people in the group they are monitoring. They aren't going to yank aside every middle eastern man woman and child and scrutinize them like you are suggesting. And even then, the people that get pulled aside will get a closer screening and if they aren't doing anything wrong, they can be on their way. This is similar to when police tell people to be on the lookout for "a 200 pound Caucasian male with a white pick up truck and a shaved head", except it's the intelligence agencies reporting it to the airports.
