Its because doctors determined that if he was extradited he would have been at a serious risk of suicide.
At least that stopped him going in 2008.

You have to remember that this started like, what, the year after nine eleven? The only reason it took this long is because the extradition process in Britain is abysmal and takes forever even on people who should be extradited immediately. At the time it was a completely reasonable response to someone hacking into the Pentagon, now it seems silly because it's been in bureaucratic hell for almost ten years.
