That's from one of the alternate futures isn't it? The one with Riker as captain. I remember there being a good reason for that, something like at a particular point in that timeline the galaxy went into constant all-out war so they haven't had the time nor resources to design or make new ships, and are essentially refurbishing every single ship that's been manufactured since that split in the timeline.
Actually I feel like I'm confusing the details for another episode, not sure which one but Guinan was aware of the timeline changing or something...
The third nacelle was a in joke in the st community because the Galaxy X was so poorly designed. In canon it is the equivalent of a third or fith wheel.
Basically in fan art if you wanted to show that a ship was bigger or better you added an extra nacelle.
Justin Lin is one of the worst high-profile directors you could pick to do this. Eugh. The man has no style.

That was Yesterday's Enterprise. It's sad I know some of the episodes by name, but anyway, the ship was normal in that.
