Potential fatalities/injuries due to health issues aren't a major issue when it comes to the safety of the vehicles. Proper maintenance and piloting are far more important.

You could restrict the use of these vehicles to people in good health; similar to how people with heart conditions aren't allowed on rollercoasters.
