Personally, I feel there are most likely two sides to Phil due to mental illness, happy Phil, and "defense mode Phil". It has been obvious for a long time that he isn't well, and he desperately needs help. I don't think he is a monster, but I think he desperately needs help.

Is it just that felt only satisfied in watching FEZ in a playthrough of the game and not paying a penny for it. Like it's a good flash game that get's money from ads which I don't click on and block.

Not only that, the issue has been made into the pink elephant in the room, where media and politicians would rather find a scapegoat to dance around this issue than tackling it head on.

I don't think Phil Fish has an actual mental illness of sorts. I think he just has terrible anger management issues.

Are they a mild or severe case? Do they also seek treatment in the form of medication or therapy for it?
Like I said a few pages back, I believe if Phil Fish is indeed suffering from some sort of mental illness, that it is currently being untreated.

Somewhere in Indie Game : The Movie he mentioned that he got into an argument with that other guy he worked with, who had enough and left (or he fired him, can't remember), and after that and because of people complaining about the game getting delayed so much he thought about killing himself. Considering this is the only time he's ever mentioned it an that it was after another one of his pissy moments I assume it was just him being really angry and trying to be a drama queen once again.

During the development of Fez in the movie, they ask him what would he do if the FEZ project fell apart, where he replied "I would kill myself", without hesitation or emotion, pauses for a few seconds, then reconfirms what he just said.

the most likely mental illness he may have is narcissistic personality disorder or traits of it
or he could be the polar opposite and have horrible self-esteem and be so insecure about himself that it makes him lash out like this and be hyper-sensitive to criticism... but i think the first scenario is more likely

A lot of people over-exaggerate and mention suicide in dire situations. How many times have you heard someone say "I was so embarassed I wished I was dead" ? Yeah. It's common.
And even if Phil Fish did have a mental disorder, it would not justify or explain most of his shitty behavior, because as far as I'm concerned, people with mental disorders that aren't crazy enough to actually be dangerous to others tend to seek help on their own, or get themselves in situations that force them to get said help. If Fish really had mental issues, chances are on top of his 30-something years of age he would already have gotten therapy at some point, and if he's still going through it you'd be really sure he would have mentioned it about a thousand times.

Looks like he is extremely impulsive too. Also by the sound of things, he seems to be blowing off his mental illness as a side note as well, which means it is probably untreated.
