Here (Florida) I always hear Micky D's
Never used Macca's, I call it a Weight Gain Center
They've been calling it Macca's in a lot of ads recently, I'd say it's more effective because they can get the product name in without a disconnect between what they say and what we say.
The fun in the name is in it being a nickname though, and if they tried to make it official, as in more than this promotional change, it'd ruin it. McDonalds is the standard on the market, the one that appeals to comfort more than edginess, don't try and make the brand cool when it's already more than successful.

I always forget that different countries have different knicknames for it, everyone gets confused when I say "brb, going for maccas run" online.

Strange, I actually live in the Sutherland Shire and I barely hear people call it Macca's at all. I'm aware of the nickname, but most people I know just call it McDonalds.

Australian English is very lazy. Almost everything gets nicknamed in such a way as to reduce the name to the least amount as syllables as possible with letters that are the easiest to pronounced.
One of the neighbouring suburb's to mine, Laverton, is referred to as 'Lavo' by everybody around here.
The 'z' thing is more affectionate with people's names, though, and it doesn't necessarily make them any easier to pronounce. One of my friends has the surname 'Barnes' and everybody knows him as 'Barnzey'.
Oh, and when we aren't calling it mickey d's, we be calling it Mack Donnals

There was already a Burger King before the US could branch to Australia, due to legal issues it had to be renamed. For a while they did call the stores Burger King, a short while after they expanded to Australia, but legal issues with the Australian corporation Burger King, forced them to have the name Hungry Jacks.

We even call it Maccas on tv ads, so I don't see any problem with this.
Also funny story. Some friends and I were at Houston airport, and we needed dinner. So one of my friends asked some random guy where "the maccas was." He replied by saying they didn't have one at the airport. We were completely confused because an airport in texas seems like a place where one would definitely have to be.
Then we realised he had no idea what the hell a maccas was.
