It did take me a while to figure it out but he went under the escelator, which underneath is rolling towards the start of the stairs. His body got trapped in between the bottom stairs and the top stairs grinding him to bits.

Don't they have a safety system that stops it when there's resistance in the machinery?

Factories and workplaces that handle big things always say to just drop the material, it can always be replaced whereas a crushed limb cannot

He didn't get grinded though. You get to see a shot of his body at the end, and while his shirt is pretty ripped on his back and he's bleeding, he's far from grinded.
Never using an escelator again, normal stairs even give you some exercise.
