In reality, most rape in prison isn't about revenge, but usually it is about dominance and power. If this guy even gets to prison they'll probably just beat the living shit out of him.

But, most likely, any decent defense attorney would be able to make a pretty solid plea for insanity. Which means he'll be put through the broken mental-health system and then let out again.

In my honest opinion, there isn't a truly GOOD answer for perpetrators of crimes where insanity is involved like this.
