Looks like she didn't have anything to hold onto other than the restraint bar once it failed. I think they should have to make a safer car design before they reopen this ride to the public.
Man, I've been on this ride so many times. I'm not even sure how you could fall out on accident. The ride doesn't really has much in the way of negative G's. If anything you're pinned to the seat most of the time.

This ride is SAFE. That car is SAFE. The whole point of a roller-coaster is to feel exposed and vulnerable; if you had 1001 restraints and a cover and a tonne of handlebars and a seatbelt and a helmet, it would kind of take away the whole fun of the ride. People ride wooden coasters because of how "out of control" they feel ; despite being safe.
What happened was a freak accident, we don't need to redesign anything.
There's no such thing as a freak accident. When someone dies, you can't just throw your hands up in the air and go "Oh well, shit happens!".
