Second, you mean an Intercontinental balistic missile that splits off to target several different targets. Third, fat man and little boy were shitty things barely able to start a chain reaction, modern Nuclear weaponry is obvously going to be a hell of a lot more powerful than it. Btw, only the US, Russia and China have decent ICBMs so unless we go to war with them for some unrelated reason, it ain't going to happen.

Most dropping a bomb on a city would require that they can fly the plane over the city without being shot down by SAMs or intercepted by fighters. A nuclear carbomb seems the most likely but they still have to actually get it to the US which isn't exactly going to be an easy task. The very obtaining of a Nuclear weapon is something that they still haven't managed (aside from Pakistan who is very much under the thumb of the US and if we went to war with them, the nukes would be under our control very quickly).

Even if one country does use a nuclear weapon, if diplomatic relations and communications are good among other countries, military entities, and heads of state in the world, global thermonuclear war may be avoided.

Nuclear war is really only a matter of time. With so many floating around it's inevitable that either a terrorist organisation will get a hold of one or a country will get backed into a corner and use one. Or another crazed hitler/kim jong will pop up and not be afraid to use nukes.

If you were to launch a nuclear attack, you'd fire all of them at once at as many places as you can, and the other people would do the same. if a tactical nuke was used against military forces, that may be a different situation.
Unless they want most of the planet to be uninhabitable, none of them will fire nukes anytime soon. Like Hypno-Toad said, they would not fire just one- they would use all of them straight away, then the other countries with nukes will probably do the same.
In the event that one nation detonates a nuclear device in an act of war or terror... That nation becomes a pile of smouldering dust. No, seriously. They'd be nuked out of existence within the day. They wouldn't last seven minutes.
