sometimes women just dont show outward signs of growth from pregnancy, this happened with my brother's ex-girlfriend's cousin
was told she was 3 months pregnant, went into labor a few hours later and had a healthy baby

A lot of female soldiers decide to take pills or have injections to block their periods whilst they're in Afghanistan.

Some pregnancies do indeed happen almost hidden. It's very rare, but it happens. Probably more common with intense physical labour. On top of that, with intense physical labour mentruation cycles can get a wee bit weird. So she might just not have noticed them very well.
Remember there being a case of a manager who gave birth on the way to a board meeting, having no clue she was actually pregnant.
