The most lethal and damaging object is the ball bearings or nails, but the secondary projectiles created by the initial blast wave are also lethal. Bits of concrete, spikes of wooden fence, high velocity shards from plastics or glass, bone fragments and things like keys/coins that might have been in people's pockets. The bomb may have been quite small, but people are throwing the word "military grade" around. It wasn't a military grade device, it was a home made device.
