Long term addiction causes necrosis, not just a one time thing and to try to get off that drug means going through a very painful withdrawal phrase for at least several months. Yeah you could make it properly in a lab, but it's homemade for a reason: krokodil addicts are too poor to buy heroin while buying common medicine is no problem. Making it in a lab and profiting from it defeats the purpose of the drug itself.
