underweight and skinny is not the same thing
one is a result of frame and fat composition, the other one is malnourishment or illness
being underweight is just as dangerous as being overweight

It depends -BMI itself isn't the best indicator of how fat someone is. Muscle tissue is heavier than fat but takes less space for instance.

That said even if she did get a sizeable portion of fat, I have pretty high doubts that it's an amount that would make her actually look fat.

It depends how much she weighted before of course.
