'May' is a lovely word in legality and politics. In this case, the Americans must be fairly sure that it was a grenade that killed the hostage - otherwise they wouldn't have mentioned it - but use the word 'may' so that if it does turn out it wasn't a grenade that killed her, they can't be blamed for providing false information.
It's a real shame that the hostage died during the rescue attempt.
But you can bet damn sure that, had they not tried to save them, they would have been killed.
