Most likely this would be caused by the private medical companies. They don't want a free choice as that will lose them business. And that is what medical treatment in the US is...a business.

My grandparents had an $8000 plan, and paid about $1000 per year for it, which was particularly high back then. In that one year, they accumulated approximately $12,000 in medical fees. The company paid it all up, just fine. One of my great grandparents were also alive at this time, and she was also covered by this plan. Just so you know, they higher the $ value of a plan (e.g. $5000 or $8000), the less expensive it is.
That's top rate. So there will be states that are paying more that you are. You have already claimed to be paying more tax that the UK, and their National Health isn't bad at all.
