Truth be told if you are crushingly poor you can sign up for care source right now and basically get free medical at supported hospitals (read: almost all of them). The only thing it won't cover is special cases, i.e. things that might require MRI's, things that require specilized treatment, etc. Of course the problem with care source is you need to apply for it at a local government agency, wait in line for several hours, and even then you might not qualify unless you are poor. And it's technically considered a form of welfare, which means you now collect welfare if you are on care source, and as such are affected by things that are wellfare-sensative (which granted, has little negative side effects other than basically getting "free" money from the government). The idea behind the "Obamacare" bill was to force people who could afford health care to get health care (anyone 33% above the poverty line, and as such people who wouldn't qualify for Care Source). With everyone paying into health care, it means the poor can more easily get coverage, it means health care premuiums go way down (because everyone is paying into it instead of select people), and it helps pay for stuff like Care Source which already exists.
