It creates enough lag to be noticeable and it has to out of principle, since it needs to calculate the difference between frames. Also the speed that it calculates this usually isn't priority since its mainly for movies and TV shows and all the have to do is introduce a delay to the audio.
Its going to vary from set to set, but I've seen it bad enough that it causes a full second delay (and it wasn't a cheap TV, it was a nearly $1000 TV). There's a reason why most sets have a "game mode" that disables it.

There was a game that did first person cover pretty well(without switching to third), can't remember what game it was though.

It was already confirmed that Cover will always be first person if you choose. You can also set it to go third person in cover.
