I have seen some that are like that but some (especially old ones) aren't. Some are wired to stop when there is like half a meter left but if you are tinkering with the box you can override it. It's pretty hard to crush yourself though, you have to hold the tool to the box so that it closes the circuit between two points, when you let go the elevator brakes and stops instantly. Some have a button that does the same thing so maybe the button got stuck or the wiring was wrong.

I've been on that ship, know almost exactly where that happened. Really sad about that guy, 66 and getting crushed like that. Fucking hell.

I'm probably going to avoid Ecstasy in the future simply for knowing about this.

Seriously though, that is incredibly graphic for local news. I wonder who signed off on showing those images. I also like the graphic imagery warning after the bloody elevator was shown.
