From what we have seen in current conflicts, and some of the stuff off-handedly mentioned by US Planners, its to be expected that 1000's will die of starvation within the first few months within big cities. Disease will spread like wildfire as the medical supplies will be mostly used up/stowed away by those involved in combat, namely the military and non-state actors. One example that comes to mind is East Ghouta: Many of the civilians resorted to keeping rats/pigeons and also growing mushrooms off of human excrement piles in order to survive. Go look up communist and especially anarchist content on the 'tubes. There are concepts like Dual Power that show us ways to subvert and replace the state locally that can be applied like creating your own daraprim at home to replace some of the state's role in healthcare or getting all your neighbours on board protecting your apartment complex to replace cops. The revolution could be looked at as a series of small events that strip away the state's power to do things. Think about how many little steps it took to create ICE and child concentration camps or how many little steps we're taking towards fascism with police militarization. In addition, especially in the midwest with growing droughts, they've been importing water. Their medical supplies are not made locally, nor are stocked in any sort of way and they're more likely to have their entire town infected than entire cities will be. The chances of a town surviving a pandemic flu is incredibly low, and that's not me saying talking out of my ass. Many towns went from hundreds to just dozens during the Spanish Flu Pandemic. They didn't have the advanced, and still don't, labs and treatment centers. Additionally, they're entirely trapped by oil, most towns are so sparsely apart that traveling on foot is a 2 to 3 day job and without gas you're ven more fucked than the Urban people.
