This would really lower down the mass KoS ingame with a realistic feeling. It would make you think twice about killing someone, making it a choice of: Do I want to have a lower sanity which would make the game harder, or kill the person and get his loot to increase my chances of survival? More ideas I have is: Bodies stay on the ground but decay over time, if your player sees a body up close, their sanity will decrease depending on what state of the decay the body is in. Murdering someone with a melee weapon would decrease your sanity more than shooting someone. Not being around players should only decrease your sanity to a certain level, IE level 0 is insanity, 50 is max, not being around players = 45 sanity which has little to no effects. Hallucinations are blurry and sort of transparent, making them easy to see as hallucinations up close but from a far they look like players and or animals. Being insane or having anxiety can make it so there is a chance of not being able to stop bleeding when you use a med kit, causing you to maybe use two instead of one. Possibly burning yourself while cooking food or smelting. Killing 2-3 people won't make you insane, because when it's a wasteland, you realize you have to do anything to survive, and that would mean killing another human being, but going on a massacre and killing every person you see would cause a lower sanity. Also, to everyone that is saying "This iz a bawd feturez becuz it will mak da game hard far me to plah it" then go play Minecraft or something. This is a great idea and before you try to push it to the side and act like it would make the game terrible, then give it a chance and if you don't like it then the dev team can remove it with the single press of the delete button.

I think it is an awesome feature! Good thinking i think they should FOR SURE implement this it will make their game stand out

Dude you need to be a DEV
This could significantly increase the difficulty of the game for an player. First off insanity after killing many people? How aren't you gonna kill many people with all the RDMing going on, this would be awful until voice is released. Also not noticing damage? How would you know when you have some dick hatcheting you in the back after you looted a location. Being left alone and all the other things, this would not make the game enjoyable this would restrict players from playing as to how they want to (Mass Murder? Lonely?) I mean you explain the simple fixes for not having or curing it but it could turn this game into a disaster, if they would consider implementing this into the game they would need an experimental group. Also think of how much coding and shit would need to be done to be able to have every player with these attributes.

This would make people think twice about spawnkilling and murdering people who are just starting to settle. Not every aspect of my idea has to be added, by the way. I am not considering loneliness in my idea anymore. Rust is supposed to be survival? It is realistic for murderers to suffer mental issues. Besides, if they got a good thing set up, they can take lithium tablets and stuff. Thought of some new ideas. Maybe if the enemy player shoots within 50 meters of you or damages you, it does not lower your sanity by killing him. I have no idea what the Rust code itself looks like, but I'd probably be able to hack together a generic prototype of this system in an afternoon once I had a design planned out. Object-oriented programming, which I assume is at least part of Rust/Unity's design methodology, makes this fairly easy because you just create as many copies of the thing (player, prop, item pickup) as you want by calling new instances of it. So, let's stick to discussing the pros and cons of the idea as it applies to gameplay, balance, and fun. How hard it is to code doesn't matter, because that's only a matter of how long it takes to get right. And that can usually be solved by hiring more good people to get it done faster if you want to spend the money. :D

Damn, Puppyxz must really hate you, sockem. Anyways, this seems like a cool suggestion, and if it was implemented, would be an interesting addition to the game.
