This is an interesting idea, makes it much more realistic, but would having to poop really make that much of a difference? No other game has touched this subject before, but I really don't think the Rust devs are aiming for mind blowing and brand new unique game play. I could see this being attempted and working if the need to poop was as serious as the need to eat.
Examine the possibility that if you keep pooping inside your home, the inside becomes a hazardous area, making it unliveable. So players will be forced to leave the safety of their homes to poop outside, now there's a huge poop pile outside of somebody's home, so you know they are active. So you sit around and wait for them to come out to poop again, you know they will come out eventually. You can't stay inside your home to poop cause you'll end up ruining the inside, so now you are forced to expose yourself to danger for the sake of your health.
This would make sense, but do we really want to have to poop in a game? The answer is YES! So long as it gets people out of their homes. It could work theoretically, but don't expect the devs to actually go through with it.
You could craft outhouses to put near your base,

Combine Pee and Poop, call it a "Waste" meter. This is survival after all, and in a survival situation you must have a plan for your bodily waste.
As off the wall as this idea is, I think it is 100% appropriate and feasible.
Make poop into "pitch" and coat your spike walls to add poison damage.
Urine could be an attractant for predators, or just as an insult gesture.

I like the idea to go outside to shit. Inside the house poo would damage you like radiation, so you are forced to go out. Awesome!
At least there has to be something that would force you to go out periodically. If you're rich on food you can stay inside almost as long as you want. Thats not good.

or maybe a little intestine graphic on the bottom left hand corner of screen with a poop running thru it. when it gets to ur anus, it starts to build up pressure turning red until it just 'splodes. so people who are champion eaters while in a gunfight will eventually have to poop, screwing up their aim when they soil themselves

This does seem like a comedy thread to me, but I will go ahead and approve this idea anyway. I'm all for realism, and I always get tired of that you can run for 10000 hours in a game and never ever have to shit and pee and so on. In normal FPS you don't eat or drink either, and Facepunch have taken that step, so why not go all the way?
Some other levels of realism that could be added; sneezes (these happen on random, and shake your view wildly, making it devastating at some point of your Rust journey) coughing (these also shake your view, but milder, but also longer. These could happen while going through radiated areas).

wow, sneezes idea is epic! i love it!
Coughs and Sneezes get +1 from me as well. Cold would obviously increase your chances of a random sneeze/cough/shiver.
Certain foods (Beans, other canned goods) could also increase chance of a random fart/burp event.
Hunger should also include the % chance to have a stomach rumble or growl.
See what brilliant ideas come from a comedy post about shit? PLEASE tell me one of the developers has at least browsed this thread, its full of game-play gold.
