Unless we make pretty specific assumptions about his history, that's all I can think of within this spectrum.
You could talk more broad aspects of our culture, and how police are conditioned to respond to situations (because the mentally ill causing a disturbance isn't unique) but that's quite a large issue and I'm not exactly sure how we can achieve meaningful change there.

Yes, it is perfectly acceptable to point out the institutional racism in america. However, not everything is racially motivated and acting as if everything is racially motivated is dangerous. Mainly because acts that do not have any evidence of racial motivation that are treated as if they where racially motivated only damage civil rights movements.
In hindsight, we now know that this person had a pipe. However, brandishing any item as if it is a gun is called simulating a weapon, as long as people think it is real, react as if it is real , it is considered real. Mainly because no one know that it is not a gun. In some of the surveillance footage screen caps in the article show it is extremely hard at a distance to see that. To me it almost looked like a small break action revolver.
If anything this highlights the horrible inequality when it comes to socioeconomic class and lack of accessible healthcare, especially mental health care.

Well yeah, no one has to die, no crime has to ever be committed, etc. if we had a perfect society.

Well, almost everyone who has mental health issues generally speaking doesn't enjoy having those issues. So creating an environment of trust of the mental healthcare system is important and could fix the problem
