They don't. The basic way the US healthcare system works is this: Insurance companies demand huge discounts. They want a price tag that says "95% off!" But the hospitals still need to make enough money to run. So, the hospitals come up with massively inflated fake prices for everything. So now instead of a $100 procedure, it's 95% off, marked down from $100,000. But only if you have insurance. If you don't have insurance, you have to pay full price.

This is the big one. If you ever accidentally go to a doctor that isn't covered by your insurance, you're completely fucked into paying full price. My boyfriend has California state health insurance, and we have to do research before going into any doctor in order to make sure we're not saddled with a multithousand dollar bill.
My health insurance covers most doctors, but unlike his there's a big premium and co-pay for basically anything, so while for him we have to do research, for me we just have to bite the bullet.

Getting insurance to everyone or regulating medical prices to be reasonable to pay out of pocket is the problem. No one deserves a 100k bill that is normally supposed to be paid by an insurance company.
