to play devil's advocate, as a performer you really shouldn't be climbing random bits of stage without clearing it beforehand. Manson probably decided to climb that prop on a whim and unfortunately it collapsed onto him. Admittedly, the prop really should've been weighed down or ratchet strapped or something - almost all set really should be secured, so it's a failure on both parts. Either way, someone's g fired.
From the title, I was expecting some Final Destination-type shit. Glad he wasn't hurt too bad.

If I was to guess, I'd be under the impression that the truss was secured using a standard square baseplate instead of a more stable crucifix style. Like this:

It's likely it was never planned to take the weight of someone pulling on it from one side, the scaf bars + props probably don't weigh too much so the square base would've been fine with no stability issues up until that point.
Manson has a past history of not making his crews life easy, [URL="https://youtu.be/Q18eYQqaK_U"]such as going over to his FOH desk and pushing it as loud as it can go[/URL] so it is a bit surprising that it wasn't sandbagged or plated crucifix style.
At a guess, Manson is likely a proper stickler for stage appearance and doesn't want anything outside of his stiff aesthetic around and the crucifix plate would probably be a trip hazard as well.
You can't really weld truss either, as you'd have to unweld it every night which would significantly effect the load bearing capabilities of it.
