It really is up to the city, since there's not really a mandate for it. Some see the risk being too high and decide to encrypt their frequencies instead. Honestly I haven't really researched whether this has made an impact at all but judging from the fact that it's still not mandated I'd assume it hasn't.

Usually companies book commercial airtime in advance, and they agree with the networks to slot them in at specific times. The network is then paid per-airing. I dunno how this works with live TV though, but I'd assume it's similar but more flexible, meaning CNN is just pushing as much of that booked airtime as possible to get as much revenue as quickly as possible.

They don't get paid per views like in Youtube, they get paid for the spot.

In nearly every event like this, some innocent bystander gets roped into the frenzied aftermath and has their life destroyed because of internet detectives chomping at bad information and misunderstandings.
