Most home barbecues are never properly cleaned. Not defending that its only piss but you are more likely to get sick from an improperly cleaned grill than piss residue in said grill.
Again, a garden hose is all you need once it cools off. Optionally a pressure washer if you are one of those folks who only ever brush the grill.

There probably aren't any real health concerns, but would you use a grill that you knew someone pissed on?

It's more a matter of principle than anything else since a grill is used to prepare food. Sorta like desecration. If the kid pissed on a car it probably wouldn't be [i]that[/i] big of a deal.
