It’s probably so cylindrical because of how cartoony the body shape is.

Although @VodMesa you could cheat a little and not follow its body type, the outfit will replace the body and only needs to fit around the neck, so all other parts are free game when it comes to shapes, it doesn’t have to be a perfect pill.
